Looking for Lan consent fourms

OK all I am looking for these ready for when i get my LAN started so that anyone under the age of 16 will be allowed into the venue. Basic it is so there parents know where they are and who to get a hold of in a problem.

Consent Form:
Due to the mature rating of some games, players under the age of 18 must submit this consent form, signed by a parent or legal guardian in order to participate in the [Event Name]

Consent to Participate and Liability Release:

I am the parent or legal guardian of _______________ (“Minor”) who is under 18 years of age.  I hereby consent to Minor’s participation in the [Event Name], consisting primarily of a video game tournament in which an entry fee is charged and cash and other prizes may be awarded to certain winners.  I am aware that the events will feature video game competitions featuring one or more video games that have an Entertainment Software Rating Board (ESRB) rating of “M” or Mature.  I am aware that this rating indicates that the ESRB, a recognized industry adopted ratings agency, has deemed the content of the video games unsuitable for children under 17 years old.  In addition, I assume all responsibility and liability for any injury, minor or serious, accidental or intentional, to Minor while at the event and/or its surrounding areas and give my permission to allow the above named minor to attend and participate in the [EventName].
